Title: The Joys and Dangers...
Post by: Ken on June 26, 2017, 07:18:29 PM
... of Game Console play.

The short version: I've sustained a potentially serious injury playing one of my console games.  :o

OK, first the back story, in two parts.
1/ My fav game to play on my PS3 is GT6 (http://www.gran-turismo.com/us/products/gt6/) and to improve game play I use a Driving Force (https://www.amazon.com/Logitech-PlayStation-Driving-Force-Racing-3/dp/B0015HYPOO) steering wheel. Because the Driving Force is several years old now, it is beginning to have a few issues, most notably the brake peddle sometimes hangs up so that you will be trying to accelerate and the brakes will be dragging. After rebuilding the peddle mechanism a couple of times I came up with the idea to use a bungee cord to help the peddle return to the brakes off position.

2/ Because of my diabetic neuropathy the feeling in my feet is very low and the feet are actually quite numb, so while I can feel large, blunt objects the touch of a small or sharp object may go unnoticed.

So, as you've likely guessed already, the end piece of the bungee cord flipped around and was sticking up towards the bottom of my foot and when I would be smashing on the brake peddle, the end of that hook was digging into the bottom of my foot!  :censored:

This morning my foot was swollen and my big toe was hurting, like a tooth ache, so Kay suggested a good soak and that did help, but she also though a good inspection was in order and that's when she discovered a big gash in the bottom of my foot, just behind the big toe. So now it's all bandaged up and I'm going to see my Dr tomorrow to see what must be done. Hopefully this dose not turn into something really bad.

BTW... looks like my game play if off for today! laughing7

Looks like you might have to stay off your foot for awhile. I hope it's nothing serious...

Going to see a podiatrist this afternoon.  :o

My family Dr thinks that the nail needs to come off of my big toe... that should be fun!  laughing7

Sorry for the delay in reporting back.

Turns out better than expected!  laughing7
After some serious trimming on the big toe nail and the end of the toe it looks like the nail may  be OK and the wound is healing nicely.
One thing though... I'm no longer permitted to do my nails on my own, seems to much damage happens when I get a pair of clippers in my hand.  :o

Thing is, the Doc was saying that I'm not to do driving games any more and I'm thinking to myself, good luck getting me to stick with that plan Doc! hahahahaha!

You need to stop rigging them...and maybe wait for your foot to heal. LOL

Turns out the foot issue (wound) still remains. Currently my Dr has me staying off the foot... no walking, standing, or any other activity that involves putting pressure on the foot.

So, for the next week and a half I'm restricted to my recliner.  :censored:

Title: My Big Toe!

Back in June 2017 I posted a topic about The Joys and Dangers of Game Console play. (https://www.ourfamilyforum.org/FamilyForum/index.php?topic=4267.0)
The topic was about an injury to the bottom of my foot and there was also a mention of the big toenail and the possibility that it might need to come off.

Well, that day has finally arrived!  :opps:

The story is long and winding, as you might imagine, but basically, I tried two different doctors in that same practice and finally just stopped because it was like I was talking, but they couldn't hear me. Then about three weeks ago I made an appointment with a new doctor in a different clinic and we set the appointment for today. This renewed attempt at getting some reasonable treatment was due to the toenail continuing to get worse over time and I felt that something had to be done with the nail.

And then...  :o
Monday of last week, (6/3) I was out in the yard doing some cleanup and mowing. The shoes that I was wearing are very hard and stiff, almost like a 'steel-toed' shoe would be and because of my feet having extremely low sensitivity I had no clue as to the damage that was being done to the nail of my big toe. The injury only came to light after I came back inside after a couple of hours and took off the shoes and then saw that the end of my sock was filled with blood.
The image above only shows the toe after returning home because there are no images of what it looked like before the surgery because it was rather gross and disgusting. The nail was hanging on by a small strip of skin on one side and the doctor didn't need to do any cutting but only had to pull it off with his fingers, including the tip of the toe as there had been some damage there as well.

Now I get to be fat and lazy for the next ten days before going back in for a checkup... he sez me has to stay in my recliner all the while.  :censored:

Don't you just love it when you think that an issue is resolved, but it comes back with a vengeance?

My original complaint was a cut on the bottom of my left foot and while this problem is on my right foot it has been caused by the same source... my driving console and the GT Sport driving game.

For some time now the gas peddle on my driving rig has been making a callous on the 'shoulder' (behind the little toe) of my right foot. This past Sunday the callous began to break apart and as it turns out there was a serious infection under the area. During an 'emergency' visit with my podiatrist, the subject of amputation even came up!  shocked003
I'm still not doing real good, with lots of swelling and redness in my foot, but with daily wound care by my wonderful wife Kay and doses of a strong antibiotic, there is great hope that all will work out well.

And now the other shoe drops... or a foot falls off, or something.

So, as I've documented before, my feet are rather numb and so things happen that go unnoticed, sometimes until way too long as in the current dilemma.

This latest bit of drama involves my other foot, and believe it or not it's also about my driving game!  shocked003
Dear reader, as you may have surmised all along, my love of my driving games is stronger than my cautionary instincts should have been and so there I was just driving away! This time I had developed a rather large callous just behind the little toe on my right foot and although I knew that the callous was there, the damage being done by the accelerator went unnoticed until it was ripped almost completely off and had become massively infected.

Then we see a series of visits to my podiatrist, with minor surgery and then daily treatments and bindings and after about four weeks while it seemed to be getting better, suddenly there was pronounced swelling of the whole foot and lots of redness, so my family Dr set me up with a wound clinic to see just what the heck is up. The wound Dr. was very knowledgeable and through and after a few minutes of him probing around, he found that the infection is down to the bones next to the toes, which means that there is a very good possibility that major foot surgery is called for.

I go in for a consultation with a surgeon on Tuesday to see what's next, hopefully, the news won't be too bad.
Are my driving game days over? Yes sadly they are over and I've already dismantled my driving rig and consul next comes selling of my equipment, including my PS4 and all the games.

EDIT: should have said "on Thursday" for the consul date.

Saving me a little toe!  ... errr, maybe.  :huh1:

Actually the news is not horrible, but it's not all that good either. My foot surgeon, Robert M Goecker, DPM ran a fresh set of Xrays and finds that infection is indeed into the bone just behind my little toe. The 'Proximal Phalanx' bone has lost about 50% of its mass due to the infection and the fix is to cut the bone about in half and remove the degraded portion. The problem at this point is that Dr. Goecker will not know for sure about the little toe until he opens up the foot, this he will do from the top of the foot even though the wound is actually on the bottom.

The surgery will be in about two weeks and then the recovery should take about 6-8 weeks, but given my history of slow healing with my foot injuries, it may take a bit longer. I'm guessing there won't be any DC Hiking (https://www.ourfamilyforum.org/FamilyForum/index.php?board=53.0) for me this year!  :hah!:

... and, speaking of COVID-19!

COVID-19 Testing
Going in for COVID testing on Monday... it's part of my pre-screen testing as we get ready for the surgery that's coming up on Friday (27th).

Saw my foot surgeon on Thursday (9th) and set a date for the surgery, it will be on July 17. There's still lots of swelling and redness so it's not looking very good for the little toe and I could tell that my Dr is thinking that it's gotta go. Got a lot of pain in my foot and the toes and it feels kind of like a really bad toothache and that's making me take more pain meds that I prefer.

My oldest son, Ken III sez, "Keep your pinky toe in a Mason jar for good luck"
And my second son, Curtis sez, "@Kenny, maybe it could be attached to his keychain like a Rabbit's foot!

My preference is the Rabbit's foot!  laughing7

Today is surgery day!
... COVID test results were negative, nice to know.
From all of my reading, this would appear to be a relatively minor and routine procedure so I'm not worried, just anxious to be done with all the drama that goes along with the day.

BTW... The old aphorism, "When it rains it pours", just might apply with my feet currently! Now the other (left) foot has become substantially swollen and discolored over the last three days, so much so that it's hard to get my shoe on. I'll show it to Dr. Goecker to see what he thinks... I hope that this will not interfere with the schedule for today's surgery.  shocked003

Please send me some prayers, I'm always in need of Spiritual help and I'm grateful for any received!

Back in my recliner and feeling tired and achy.  laughing7

The news on the toe is good, for now, and the 'for now' is because that depends on how the recovery process goes... six to eight weeks according to my Dr.

So, a couple of pics for your entertainment.

This longer view of the foot after we returned from the surgical center and got settled into my recliner... this recliner will be my home for the next eight weeks.  :'(

Not really crying or whining about it, but it will require some determination and intestinal fortitude on my part to not start cheating the process, but I will be strong.
... note the compression on the left leg, this is the short term fix for the swelling in the left foot and ankle, may need to do more later.

Yes, I know that this one is gross, but it gives context for why this surgery was needed.

That comes from my diabetic nephropathy and how it makes my feet numb.

Went to our local ER yesterday and stayed about 5 1/5 hours trying to find out why my left foot and ankle are so swollen, red, and painful, so much so that I can hardly walk even with a walker. Kay posted this on FB recounting her part of the experience.

Quote
Yesterday was a awful day.Ken's left leg has been bruising and swelling since two weeks ago. His Foot surgeon saw it before surgery and said we needed to watch it and for him to keep the Ted hose on, so we have. When I changed his Ted hose yesterday, his ankle was swollen and the back of his ankle was bright red and very hot to touch and very painfull. I had him email our Dr., who wasn't in and someone else was supposed to get it touch with us, but didn't so we went to Forsyth Hosp. ER. Never again. I could not go in with him.I sat in the car and waited for him for 5 1/2 hours.I guess they were really busy with heart attacks and they came first. We were so afraid of a blood clot.It was 97 degrees out and I sat with the air conditioner on and then turned the car off.I sat there turning the car on and off for 5 1/2 hours. They took Ken for blood work and for Xray and Ultrasound. He would call me when he could, but it was nerve wracking. He called and said he was finally ready and would call me when they released him. That took at least 45 minutes.I wasn't able to pee even as they would not let you in the hospital. We made it home by 10:20 PM. No diagnosis given. He is to make an apt with our regular Dr.If anyone had seen the changes his left leg has gone through in the last two weeks, you would have been very suspicious also.

Went in for my one-week post-op checkup today and the foot is looking good, most of the swelling is gone and the little toe is recovering really well. Dr. Goecker knows his craft, he sewed up the top of my foot with about a dozen stitches that were very small and closely packed, so any scar is likely to be practically invisible.

Wow!
It feels strange to open up this topic again!

Me getting ready to go under the knife again!  shocked003
It's just a minor out patience surgery on my toes on the right foot to correct an issue with the way they work together. (they sometimes cause injuries to each other.) It will be on Friday the 9th.

After doing a bit of research this article seems to best describe whats going on:
... disregard this link and quote, with further research I found another article with better information. It's in my next post after this one.
Hammer toe Surgery: What to Expect (https://www.healthline.com/health/hammer-toe-surgery#complications)
Quote
Joint resection

In joint resection, an incision is made on the top part of the toe to reach the ligaments and tendons underneath. The ligaments and tendons may be cut to help straighten the toe. The end of one bone is taken off so that the toe can be fully extended. Small metal pins or rods are often used to keep the toe straight during healing. The pins are usually removed a month or so after surgery.

... this article more closely described the procedure.

What is Tendon Release Used to Treat? (https://www.reboundmd.com/news/what-tendon-release-used-treat)
Quote
Tendon release, also known as tenotomy, is a surgical procedure that involves cutting through or disconnecting a tendon to allow for a greater range of movement.

The procedure is used to relieve tight or shortened muscles. In some cases, the tendon is re-routed to maintain muscle function.

This procedure is usually reserved for conditions that don’t respond well to conservative nonsurgical treatments. Tendon release surgery is ideal for infants with clubfoot, or for older patients who develop tightened or shortened muscles due to cerebral palsy or other neuromuscular disorders. It is also used for a number of other common conditions including hammer toe, tennis elbow and injuries to the Achilles tendon.

... I don't have hammer toe, but it's kind of like the opposite of that condition.

Got the foot cast removed and my doctor was well pleased with the results and said that I can return to full duty again. No restrictions on standing or walking and that makes me a happy boy!  laughing7

Of course there always has too be the obligatory gross pix showing the new stitching.
